Learn About AFib Warning Signs and Symptoms

Understanding Atrial Fibrillation: What Happens in Your Heart

Atrial fibrillation, commonly called AFib, is a condition where the heart's upper chambers (called atria) beat in an irregular or chaotic pattern instead of in a steady rhythm. To understand this, it helps to know how a healthy heart works. In a normal heart, electrical signals travel through the heart muscle in an organized way, causing the chambers to squeeze and pump blood efficiently. These coordinated contractions happen about 60 to 100 times per minute at rest.

When someone has AFib, the electrical signals in the atria become disorganized and rapid, sometimes beating 100 to 175 times per minute or more. This irregular beating means the atria cannot pump blood effectively into the lower chambers (ventricles). The heart's rhythm becomes unpredictable—sometimes fast, sometimes slow, and often irregular. Think of it like a drummer who suddenly loses the beat and starts playing randomly instead of keeping time with the rest of the band.

According to the Centers for Disease Control and Prevention (CDC), about 2.7 to 6.1 million people in the United States have AFib, though many may not know it yet. The condition can be temporary (paroxysmal AFib), persistent, or permanent, depending on how often the irregular rhythm occurs and whether it returns to normal on its own. AFib becomes more common with age—while it can occur in younger people, it is especially prevalent in adults over 65. In fact, the American Heart Association reports that about 9% of people over age 65 have AFib.

The reason AFib matters is that irregular heartbeats can lead to complications. When blood does not move efficiently through the heart, it can pool in the atria and form clots. These clots can travel through blood vessels and potentially lodge in the brain, causing a stroke. This is why recognizing early warning signs is important for anyone at risk. The good news is that numerous treatments and management strategies exist to help people live well with AFib.

Practical Takeaway: AFib is an irregular heartbeat that affects millions of Americans. Understanding that your heart's rhythm has become chaotic, rather than steady, is the first step in recognizing whether you might need to see a doctor about your symptoms.

Common Warning Signs You Should Not Ignore

The early warning signs of AFib vary from person to person. Some people experience obvious symptoms, while others have no symptoms at all and discover AFib only during a routine medical checkup. Knowing what to watch for can help you recognize when something feels different in your body and encourage you to contact a healthcare provider.

One of the most common warning signs is a fluttering sensation in the chest or heart palpitations. Patients often describe this as feeling like their heart is "racing," "skipping beats," or "flip-flopping." The fluttering may last for a few seconds, several minutes, or even longer. Some people say it feels like a bird trapped in their chest or like their heart is pounding so hard they can see their shirt moving. Others notice that their heart seems to be beating irregularly—not in a steady pattern but in an erratic, unpredictable way.

Shortness of breath is another significant warning sign. People with AFib may feel like they cannot catch their breath, even when sitting down and not exercising. This happens because the heart is not pumping blood efficiently, so the body is not getting enough oxygen. The shortness of breath might occur during physical activity (like climbing stairs or exercising) or, in some cases, even during rest. Some individuals report that lying flat makes it worse, and they feel more comfortable sitting up.

Fatigue and weakness are also common warning signs that people sometimes overlook. You might feel unusually tired even after getting enough sleep, or you might notice that activities you normally handle easily—like grocery shopping or playing with grandchildren—now leave you exhausted. This fatigue occurs because your heart is working harder to pump blood throughout your body, and your muscles and organs are not receiving oxygen as efficiently as they should.

Additional warning signs include dizziness or lightheadedness, particularly when standing up quickly or during the irregular heartbeat episodes. Some people experience chest discomfort or pressure, which they might initially worry is a heart attack but turns out to be AFib. Anxiety is another symptom some patients report, likely because the racing heartbeat triggers a stress response in the body. A small percentage of people experience fainting episodes (syncope), though this is less common.

It is important to note that having one of these symptoms does not automatically mean you have AFib. Many other conditions can cause palpitations, shortness of breath, or fatigue. However, if you notice these warning signs, especially if they are new or occur repeatedly, contacting your doctor for evaluation is the appropriate next step.

Practical Takeaway: Common warning signs include heart palpitations, shortness of breath, fatigue, dizziness, and chest discomfort. If you experience any of these symptoms regularly or if they are new for you, write down when they occur and mention them to your doctor.

Risk Factors That May Increase Your Chances of Developing AFib

Certain conditions, habits, and characteristics increase the likelihood that someone will develop AFib at some point in their life. Understanding your personal risk factors can help you stay alert for warning signs and take steps to reduce your risk through lifestyle choices.

Age is one of the strongest risk factors for AFib. The condition becomes increasingly common as people get older. According to the Framingham Heart Study, a large long-term research project, the lifetime risk of developing AFib is about one in four for adults age 40 and older. This means that roughly one out of every four people over 40 will develop AFib at some point. While AFib can occur in younger people, it is far more common in those over 60 or 65.

High blood pressure (hypertension) is another major risk factor. When blood pressure stays elevated, it forces the heart to work harder and can damage the heart tissue, particularly in the atria. This damage can disrupt the electrical signals that control your heartbeat. Studies show that people with high blood pressure are about 1.5 times more likely to develop AFib than people with normal blood pressure. Managing blood pressure through medication, diet, and lifestyle changes may help reduce AFib risk.

Heart disease is a significant risk factor. People who have had a heart attack, have valve problems, heart failure, or other structural heart conditions face a higher risk of developing AFib. The damaged or weakened heart tissue can interfere with the normal electrical signals. Similarly, people who have had heart surgery also have increased AFib risk. Thyroid disease, particularly an overactive thyroid (hyperthyroidism), can trigger AFib because excess thyroid hormone speeds up the heartbeat and can cause irregular rhythms.

Chronic lung disease, including chronic obstructive pulmonary disease (COPD) and sleep apnea, is associated with AFib. Sleep apnea, in particular, is increasingly recognized as an AFib risk factor. During sleep apnea episodes, breathing stops repeatedly, which lowers oxygen levels in the blood and can trigger irregular heartbeats. Research suggests that treating sleep apnea may reduce AFib episodes in some people.

Lifestyle factors also play a role. Heavy alcohol consumption is linked to AFib—people who drink excessive amounts of alcohol have higher AFib risk. Smoking damages blood vessels and the heart, increasing AFib risk. Obesity puts strain on the heart and is associated with increased AFib risk. Lack of physical activity, uncontrolled diabetes, and chronic kidney disease are additional risk factors. Research from the American Heart Association shows that maintaining a healthy weight, exercising regularly, managing blood pressure and diabetes, and limiting alcohol intake may reduce AFib risk by up to 62%.

Some people inherit a genetic tendency toward AFib, meaning it runs in their family. If a parent or sibling has AFib, your risk is somewhat higher than average. Additionally, certain races and ethnicities have different AFib rates—studies show that Caucasian individuals have higher AFib rates than African American, Hispanic, and Asian populations, though the reasons for these differences are not completely understood.

Practical Takeaway: Review the risk factors listed above and identify which ones apply to you. Share this information with your doctor, and ask what steps you can take to reduce your personal AFib risk. Many risk factors—like weight, exercise, alcohol consumption, and blood pressure—are within your control.
